SERVICES TO PRIMARY CARE PROVIDERS
- Arrange follow-up appointment prior to patient discharge from facility.
- Facilitate direct admissions bypassing ER/ED’s in the Hospital and Skilled Nursing Facilities to help minimize costs.
- Notify PCP point of contact(s) for discharge disposition and discuss appropriateness.
- Record transfer for ALL PATIENTS: Hospitals. Skilled Nursing Facilities To best facilitate Transition of Patient Care back to their PCP and the community setting.
- Appropriateness of inpatient consultations with preferred partners.
- Our Providers engage in end-of-life and advanced directives conversations when necessary and facilitate plan for the patient and patient’s family/loved ones.
